School Sponsored Sport/Activity Physicals

Dear Parents,

 

It is that time of year again! We will be providing complimentary sports physicals for our athletes for the
2025-2026 academic year. This will all take place on June 5, 2025. Please remember that all athletes are required
to obtain a sports physical annually, in accordance with PIAA regulations. We encourage you to take advantage of
this opportunity for fall, winter, and spring sports. Completing this process through the school will help you avoid
any last-minute complications or trips when your child’s season begins. This will also ensure one less item on your
to-do list.

We will also be providing our annual impact test (baseline concussion assessment). This test is required every two
years; typically, if you begin playing sports in 7th grade, it should be completed in odd-numbered years. However,
we can assist you in finding that information, so please feel free to inquire when scheduling!
Sports Physicals: Tri Rivers will be conducting the physical examinations. Dr. Miller and the Physician Assistant will
perform two physicals every 5-10 minutes. Students are required to have the Section 5 (Pre Health History
Questionnaire) fully completed and signed by a parent or guardian. They should bring this form with them for
review. Additionally, students must present Section 6 (Physical Examination Sheet), which will be completed by the
physicians, indicating whether they are cleared, cleared with exceptions, or not cleared, and this form will be
returned to the students.

Impact Test: This is an online assessment that will be conducted in the computer lab. The test requires
approximately 25-30 minutes to complete. We can accommodate around 15-20 appointments every half hour.
Should there be a lengthy wait for physicals, we may escort students to the computer lab to complete their Impact
Test prior to returning to the line for physicals.
You may begin scheduling appointments today, we will be accepting appointments until the day before or until all
the appointments are filled!

We recognize that these appointments are scheduled during a half school day, but we have developed a plan to
manage this effectively. Appointments for students in grades 9-12 will take place from 8:30 AM to 11:00 AM. Staff
will receive a list of students scheduled for appointments and will allow them depart to the auditorium based on
their appointment times. We will provide direction to ensure a smooth process. From 11:00 AM to 11:30 AM,
there will be a break for lunch. During this time, junior high students can arrive for their appointments. Those with
appointments will be permitted to ride the bus after school to the high school. Upon arrival, they will check in at
the registration table in the auditorium, find a seat, and wait to be called. Appointments for both high school and
junior high students will occur from 11:35 AM to 3:00 PM. After the half day concludes, all students will need to
arrange their own transportation home. They may walk home with a parent note or be picked up by a parent or
guardian. Please note that there will be no activity bus provided that afternoon.

If any students have an earlier appointment but cannot be picked up until later in the day they may stay in the
auditorium, NO STUDENTS ARE PERMIITTED to walk the halls unless they are going from one line to the next.
We thank you all for your patience and cooperation during this hectic schedule, but we will make it a smooth day
