All documentation must be competed and board approved prior to any coach working with our student athletes.  

 

All clearances must be within one (1) year of board approval date. 

 

TB Test must be within two (2) years of board approval date.  

 

Please make sure to complete the cover sheet (checklist) and write a current email address legibly.

Volunteers do not need to get their finger prints taken (Act 114) IF they have lived in the state of Pennsylvania for the last 10 years prior to board approval.  If the volunteer has not lived in the state of Pennsylvania for the last 10 years, the Act 114 must be completed and turned in with the rest of the needed documentation.
