Please see below for a New Student Packet for enrollment to the Slippery Rock Area School District. Meal, Work Permit, and Vo-Tech Applications. As well as, a Transportation Residency form, a Transcript Request form and a College Visit form.

Please complete the New Student Packet including the Free & Reduced Lunch Document if applicable. Packets are available on our website or in the SRAHS Guidance Office. Please note that a birth certificate, proof of residency, and immunizations are also required. If your student has previous discipline occurrences, the Affidavit for Parental Registration Statement also needs notarized.

Work Permit Application and Birth Certificate are required to apply for a student's work permit. Students can visit SRAHS Guidance office during the school day to obtain this form and to process the work permit. You can also submit paperwork electronically to [email protected]. Upon completion the student must be present to sign.

Please fill out the attached form, that refer to you, and email it to [email protected]
 
Hand carry your College Visit Form to you college visit. Upon return to school, hand in to Ms. Marini for attendance purpose at the Main Office.
Complete the Transportation Residency Change Form and return to [email protected]
